Only students that are 15 years or younger need Work Permit.
You may pick up an application for a work permit on the counter in the Main Office before or after school.
Any MPS student seeking a work permit must present the following:
- Proof of Age: Birth Certificate, Baptismal Record, State of Wisconsin ID Card or Drivers License
- Letter from the Employer: Stating the intent to employ the student along with job duties.
- Parent/Guardian letter of permission
- Social Security Card
- Work Permit Fee $10.00 (Please have the exact amount as we do not have change)
No work permit can be issued without these items as required by the State of Wisconsin
